HOURS

37 ½ hours per week flexibly to suit needs of individuals and service (including evenings, weekends and public holidays)

Initial £ 13,000; rising to £ 13,750 (pending satisfactory completion of first 9 months)

ANNUAL LEAVE

37 days leave (incorporating 25 days annual leave,

plus 12 days public holiday) for full time staff.

Post summary

To provide high quality support to individual and families who are referred to the service for support.  

 

Specific duties

•1.                  Provision of Support

  • Provide support in response to identified needs as documented in each person's Support and Development Agreement (SDA).
  • Provide support in a way which equips individuals with the confidence and skills to live as independent a life as possible.
  • Document support provided within the person's SDA in line with organisational standards.
  • Act as named worker for a number of individuals or families.

 

•2.                  Communication

  • Treat individuals, families and carers with dignity and respect at all times.
  • Communicate with supervisor and/or Line Manager regarding any support needs or concerns.
  • Assist and encourage individuals to be involved in their support and moving on plans.
  • Actively contribute to effective team-working in the service.
  • Act in a professional manner at all times when dealing with external agencies.
